Transition Needs Survey

The transition needs survey (“Transition Team Survey”) includes all transition skills for a level on a single page. A simple system is used to score the survey, which allows the IEP team to focus discussion on identifying a student’s transition strengths, needs, and priorities. The case manager is responsible for keeping a master completed survey that becomes the basis of the student’s transition plan.
