Transition case managers have four main responsibilities in guiding there student’s IEP and helping them to achieve their postsecondary goals. First, they expect IEP teams to play an important role in guiding students to make a successful transition to adult life. As part of this expectation, they look for tools and methods to improve communication with the IEP team. They also share the IEP and regular progress reports, so as to encourage input and suggestions. Second, Case Managers help students to explore and identify their postsecondary goals. As required by IDEA 2004, the Transition IEP requires that students have goals related to employment, education and training, and independent living. Effective case managers take this responsibility to heart and are constantly looking assessments, conversations, and other methods to guide students to explore their wants, interests, strengths, and needs. They are also looking for ways to help their students make real-world connections between what they are learning and how it will help them in the future. The third role involves writing the IEP in a way that helps students to reach their postsecondary goals. In order to do that, they use a variety of age-appropriate Transition assessments as the basis of the IEP. They identify important things like student needs, tasks that must be completed to achieve the goals, and courses that will help the student to reach their goals. They also combine all pertinent information into a comprehensive and meaningful Transition plan. Lastly, case managers are responsible to manage the transition plan through a compliant IEP. This means they establish priorities, responsibilities, and a timeline for meeting those responsibilities. They also share the procedures and timelines for completing the IEP (plan) with the IEP teams. It is also important that case managers clearly communicate the tasks that the IEP team members are responsible for. Case managers also track day-to-day completions of transition plan responsibilities, update the IEP team on progress, and include a list of courses the student has taken to help reach their postsecondary goals.
